The following information is important to assist you in completing the steps for Alaska Chiropractic Clinical Assistant Certification through the CTA Program and maintaining a certification in good-standing.
By reading and typing your name at the end of this online form, you are acknowledging that you are now aware of these steps and the information presented below.
I understand that as soon as my exam answers and proctored session have been reviewed, I will receive email notification of my exam score (typically within 72 business hours).
I understand that the CTA Program will email a personalized certificate with my name and Alaska CCA certification expiration information. Alaska CCAs are encouraged to display their CCA certificate within the public view in the practice where they perform CCA services.
I acknowledge that I must begin to log my practical clinical experience (up to 1200 hours and no less than 300 hours) to submit for full certification within one year of successfully passing the Alaska – CTA Examination.
I recognize that I may access downloadable versions of the following items in my CTA online account to assist in logging and attesting to my clinical experience for full CCA certification:

- Alaska Clinical Experience Performance Report
I also acknowledge that upon completion of the clinical experience hours, I must submit to the CTA Program, a CCA Certification Renewal Application along with a copy of my clinical hours log and a signed Clinical Experience Performance Report for review. I understand that subsequent renewals will cover a two-year period based on a biennial schedule and that during each 2-year renewal period, I must complete at least six hours of continuing education every two years in relevant topics to meet recertification requirements and maintain their Alaska CCA status. (See the Alaska Handbook for continuing education requirements and considerations.)
I acknowledge that I am responsible for retaining my certificate of attendance for each approved seminar in which I participate as verification, in the event I am audited.
Lastly, I understand the importance of keeping my contact information current with the CTA Program and the Alaska Chiropractic Society in order to receive correspondence regarding certification renewals, continuing education opportunities and other important notices to avoid unnecessary fees and/or risk to certification.
