Performance Report
As soon as your exam answers and proctored session have been reviewed, you will receive
email notification of your exam score (typically within 72 business hours). Additionally,
we will mail your certificate verifying completion of your CTA 50-Hour instruction
and your pass letter verifying your completed TN CTA State Exam with minimum score of 75 with the CTA Seal to your home address.
IMPORTANT: I understand that I may not begin performing any CTA duties in the office until I have knowledge of passing the exam. Only once I have been notified of a successful exam score will I be eligible to begin accumulating
the required 1200 hours of clinical internship performing chiropractic therapy assistant duties under direction of a supervisor for the purpose of receiving practical training in providing physical agent modalities and rehabilitation. I understand that
only CTA related duties are acceptable and that hours devoted to other office positions are not to be included.
Per section 0260-05-.10 of the TN Rules Governing Chiropractic Therapy Assistants,
intern supervision is for the purpose of training or teaching, by an approved supervisor who monitors the performance of an individual. The supervisor provides regular documented face-to-face guidance and instruction with respect to the skills and competencies
of the person supervised. Per section 0260-05-.01, supervision is defined as the ongoing, direct review, for the purpose of training or teaching, by a licensed chiropractic, medical or osteopathic physician, or chiropractic therapy assistant supervisor who
monitors the performance of an intern.
I understand that during the time I am working in my clinical internship, I am exempt from the certification requirements, but only for a
period not to exceed one (1) year from the date of examination, unless an extension is granted by the Board of Examiners.
I acknowledge that I am aware of the CTA licensure application and process. I understand that as of January 1, 2018, the TN Board of Examiners will only accept applications submitted online. I have noted there is a list of items
that must be submitted along with the CTA licensure application, including the exam pass letter and certificate which are mailed after the online exam is reviewed. I understand that I must also include a copy of my internship log and performance report for
the Board of Examiners’ review.
Effective June 1, 2006 applicants for initial licensure in Tennessee (not renewal or reinstatement) must obtain a
criminal background check. Contact the TN Board of Examiners for instructions prior to submitting your license application.
I am aware that as a licensed CTA I am required to have six (6) hours of continuing education per calendar year
beginning the year immediately following the initial year of licensure. I have also been informed that I must attend a 2-hour, one-time only New Licensee seminar sometime within the first year that continuing education is required.
I acknowledge that I am responsible for retaining the certificate
of attendance for the 2-hour seminar and each 6-hour state approved seminar that I attend, as verification for the state, in the event I am audited.
I acknowledge that the contact information for the Tennessee Board of Chiropractic Examiners (TBCE) is on the license application. I also understand the importance of keeping my contact information current with the TBCE and the TCA
in order to receive correspondence regarding license renewals, audits and other important notices to avoid unnecessary fines and/or risk losing my license.
Lastly, I agree to read through the CTA Rules accessible in my online account in their entirety in order to be fully informed of the state requirements for all those holding CTA licensure.
